 No deductible 

Copay corresponds to plan number for office visits and generic formulary prescriptions.

Hospital Services Coverage:

                100% with Plan 5

                $300 per admission for Plan 10

                $500.00 per day with Plan 25

Unlimited lifetime maximum benefit.

Must go through Primary Care Physician

No pre-existing conditions limitations

Deductibles vary with plans:

                $250 ded. With Plan 250

                $500 ded. With Plan 500

                $1000 ded. With Plan 1000

                $2400 ded. / $4800 fam. With Plan 2400

General office visits have co-pays in all but the 2400 PPO plan, however, they are not applied to deductible.

Formulary prescription copays are $10for generic drugs in all PPO Plans except the 2400 plan.  The 2400 plan generally covers 80% of the generic drug.

PPO Plans are with Blue Shield of California

Out of network benefits available.

401 K Plan

Plan allows pre-tax deductions up to 20% of salary for investment in 401(k) Plan. 

Preferred Healthcare matches 50% of the employees' first 2.5% of contribution.

6 year vesting schedule for employer match.

Enrollment allowed quarterly (calendar yr.).

Deferral % change allowed each payroll

Eligibility Requirement: All employees 21 years of age or older.

Direct Deposit

Our system allows deposits in up to 4 different bank accounts. Auto-deposit kicks in the second payroll after entry.  Auto-deposits are generally posted the Thursday prior to payday...allowing you access to your pay first thing Friday morning.
